Who is Covered?
All members of the organization are covered and must be included in the Policy. Unpaid supervisors and advisors (e.g. adult leaders and committee persons) of the organization may also be covered at the same low premium; but in the event coverage is desired for these people, all must be included. It is not necessary to name each person covered, but a premium must be paid for each.
New members joining the organization while the Policy is in effect will be covered automatically until the Policy expiration date - without additional charge.
When Are They Covered?
Death, Dismemberment and Medical Expense losses are covered if they occur directly and independently of all other causes as a result of an accident which takes place during the Policy period. The accidental injury must be sustained by an insured member while participating in an approved and supervised activity of the organization or while traveling directly to or from such activity
Description of Coverages
Accidental Death and Dismemberment - Pays a lump sum benefit for loss of life, limbs or sight occurring within 180 days of a covered accident. If more than one loss results from the same accident, the largest applicable benefit will be paid.
Accident Medical Expense - Subject to the maximum benefit amount selected, pays the reasonable and customary expenses* (in excess of a deductible, if any) for necessary medical or surgical treatment, services or supplies if the first expense is incurred within 52 weeks of date of accident. For any one accident, all covered expenses will be paid up to the maximum amount of the plan selected if incurred within two years from the date of accident.
* "Reasonable and Customary Expenses" means the amount of such expenses which are not in excess of the average charges made for such medical or surgical treatment, services or supplies in the locality where the treatment, services or supplies are received, taking into consideration the nature and the severity of the injury.

Note: Lower rates are available for groups with over 500 members; contact your reporting offices.
Excess Coverage
Lower rates are available if the accident medical coverage is purchased on an excess basis, over and above group, blanket, or franchise health insurance coverage; other group hospital or medical service plans, and prepayment coverage; any coverage under labor management trustee plans, union welfare plans, employer organization plans, or employee benefit organization plans; coverage under any governmental programs or coverage required or provided by any statute, except Medicaid; and automobile reparations insurance (no-fault) coverage.
Please note that any amounts paid by another "Plan" as defined above cannot be used to satisfy any deductible under our Policy. The excess coverage rider may be used only on a non-contributory basis.
General Exclusions and Limitations
The Policy does not apply to: (a) intentionally self-inflicted injury, suicide or attempted suicide; (b) injury sustained while in or on any aircraft or in falling or descending therefrom or therewith, except while a passenger in or on, boarding or alighting from, an aircraft operated by a passenger airline on a regularly scheduled passenger trip over its established route; (c) repair or replacement of existing dentures, partial dentures, braces, fixed or removable bridges, or other artificial dental restoration; (d) repair, replacement, examination for prescription or fitting of eyeglasses or contact lenses; (e) repair or replacement of artificial limbs or orthopedic braces; (f) injury sustained as a team member while practicing for or participating in any inter-scholastic sports event, or traveling to or from such practice or participation; (g) sickness or disease or medical treatment thereof, except pus-forming infection which shall occur through an accidental cut or wound.
Underwriting
Complete the request form, taking into consideration the following:
- That the risk is an eligible group (if in doubt contact your reporting office);
- That the Policy is not being issued primarily for organized sports coverage.
Examples of some of the groups which qualify for this program are: Boy Scouts, Cub Scouts, Other Scouting Groups, Columbian Squires, DeMolay, Future Farmers of America, 4-H Clubs, Hi-Y, Camp Fire Girls, Girl Scouts, Blue Birds, Brownies,Tri-Hi-Y, Job's Daughters, Pathfinders,Young People's Church Groups, and Rainbow Girls.
Other substantially similar young people's groups may qualify. The mention of these names does not mean to imply endorsement of this program by any of these organizations.
This program is not available for drum and bugle corps, marching bands, boys' clubs, community centers, amateur theatrical groups, police athletic leagues or any youth group primarily organized for play or practice of athletics, nor for nursery schools, government sponsored economic opportunity programs or groups with a membership comprised primarily of adults. It is also not our intention to cover ski trips under this program. For accident insurance designed to meet the needs of groups such as these, The Hartford offers a Special Risk Accident Policy and a Sports Accident Policy.
General Remarks
The total policy premium is payable at the inception of the Policy. No refunds will be made for members who leave the organization before the Policy expires, and no additional charges will be made for new members.
A check or money order for the premium should accompany the request.
The minimum premium for each policy is $150. The Policy may not be canceled by either the Company or the policyholder.
The Policy provides ACCIDENT insurance only. It does NOT provide basic hospital, basic medical or major medical insurance as defined by the New York State Insurance Department.
